Lithrim is the execution integrity platform for healthcare AI agents. We verify what AI scribes, triage bots, and scheduling agents write to clinical systems, not just what they say. Every clinical artifact (SOAP note, referral letter, prescription, scheduling action) is checked against the source conversation for faithfulness, completeness, and safety. Each one gets a verdict (PASS, WARN, or BLOCK) before it can be written to the electronic health record. Lithrim catches wrong dosages, fabricated medical history, missed allergies, wrong laterality, incorrect codes, and missed escalations.

Our Mission

Verify what AI writes to clinical systems before it becomes a clinical error. Healthcare AI agents are transforming how clinical documentation gets created. Scribes generate SOAP notes. Triage bots route patients. Scheduling agents book appointments. But these agents do not just talk; they write to the electronic health record. A perfect-sounding conversation can produce a note with a fabricated allergy, the wrong dosage, or a missed escalation. How Verification Works

Three layers, end to end. Test before you ship, verify on every artifact, and ship a cryptographic audit trail every regulator can replay.

Pre-launch eval. Test every model update against curated failure-mode packs (wrong dosage, fabricated history, missed allergy, PHI). Verdict-match accuracy with pass, improve, or block thresholds. Side-by-side version compare to catch regressions before customers do.

Runtime verification. A three-judge council and a structural validator run on every artifact. Audio against text, not text against text. Faithfulness, Completeness, Safety, and Structural pillars decide Pass, Warn, or Block before the artifact reaches the EHR. Structural validates against your FHIR profile or custom JSON spec, the schema your hospital actually enforces.

Per-finding audit chain. Every flagged finding ships an eight-link evidence chain, from the audio segment to the final verdict, replayable from a hash receipt on demand. The audit trail your hospital buyer's regulator asks for, ready when they ask.
